Job Summary: We are seeking a dedicated and compassionate Nurse Navigator to join our team. The Nurse Navigator will play a critical role in assisting with care plan meetings, addressing clinical issues, and managing care client cases within our network. This position involves coordinating care and services, conducting assessments, and facilitating communication between healthcare providers, patients, and their families. The role is primarily virtual, involving regular interactions with hospitals, nursing homes, and family members to ensure seamless care transitions and optimal patient outcomes.

Key Responsibilities:

Care Coordination and Meetings:

  • Assist with care plan meetings to discuss and navigate clinical issues and client cases.
  • Utilize clinical nursing knowledge to participate in and facilitate care client discharge or care plan meetings.
  • Coordinate and attend virtual meetings with hospitals, nursing homes, and family members to ensure cohesive care plans.

Client Assessments:

  • Conduct initial and ongoing care assessments for new and existing clients within the network.
  • Call and assess the needs and situations of new referrals to determine appropriate care plans.
  • Complete comprehensive new care client assessments to establish a baseline for care needs.

Case Management:

  • Help manage care cases, including coordination with Piper and the care team.
  • Follow up on care plans and update them as needed based on client progress and changing needs.
  • Perform weekly reviews of cases with Piper and the care team to ensure continuous quality care.

Business Development:

  • Help identify and suggest new community and professional providers for our provider network
  • Sit on meetings to introduce, assess and or vet network provider candidates

Communication and Collaboration:

  • Maintain effective communication with clients, families, and healthcare providers using various platforms (Click Up, Practice Better, Email, Dial Pad, Slack).

  • Coordinate care services and resources to support client health and well-being.

  • Facilitate smooth transitions of care by addressing any barriers or challenges promptly.

Requirements:

Education and Licensure:

  • LPN or RN.
  • Bachelor of Science in Nursing (BSN) preferred.

Experience:

  • Minimum of 2 years of clinical nursing experience.
  • Previous experience in care coordination, case management, or similar roles preferred.

Skills and Competencies:

  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Proficiency in using healthcare-related software and communication tools (Click Up, Practice Better, Email, Dial Pad, Slack).
  • Ability to work independently and collaboratively in a virtual environment.
