A1 Service Coordination provides support to individuals enrolled in the NHTD/TBI Medicaid Waiver program. Our team assists with securing initial enrollment, maintaining annual enrollment, facilitating discharges from nursing home placements, and securing necessary supports and services to prevent nursing home placement. We help individuals access waiver and non-waiver services to maintain health, safety, and well-being while promoting independence in their homes and communities.

Role Description
This is a full-time remote role for a Service Coordinator at A1 Service Coordination. The Service Coordinator will be responsible for assisting individuals with accessing and maintaining waiver services, making referrals to community providers, and ensuring ongoing benefits and entitlements. Daily tasks include coordinating services, communicating with clients and providers, and managing operations to support individual goals and independence.

  • Ability to work independently and remotely
  • Knowledge of Medicaid and other benefit programs
  • Experience in healthcare or social services field
  • Bachelor's degree in Social Work, Healthcare Administration, Psychology, or related field
