A charity is seeking a Senior Hub Coordinator to support the delivery of home-based end-of-life care across the London and Home Counties region. Working remotely, you’ll play a vital role in coordinating services for patients and families while supporting a busy clinical care team.
Key Responsibilities:
- Act as the primary contact point for services across the region.
- Coordinate referrals, manage shift schedules, and maintain accurate care records using platforms like EMIS and SystemOne.
- Liaise with care teams, referrers, volunteers, and families to ensure seamless service delivery.
- Work in collaboration with other hub coordinators to manage operations and escalate clinical queries where necessary.
- Deliver excellent service to internal teams and patients during and after their care experience.
Role Details:
- Full-time, 37.5 hours per week
- Shift pattern: 5 days/week between 08:00–22:00 (including weekends)
- Salary: £24,625 – £25,674 per annum (Band 3, AFC-aligned)
Ideal Candidate:
- Customer service experience, ideally in a healthcare or care coordination setting.
- Strong IT and communication skills.
- Meticulous attention to detail and data accuracy.
- Self-motivated, professional, and able to handle sensitive interactions.
- Strong multitasking and organisational abilities.
- Experience in shift-based team support or healthcare logistics is desirable.
