This role involves outreach to both members and providers to promote preventive health services and improve overall care coordination. The position is remote, with a preference for candidates residing in the Central Time Zone, and offers a pay rate of $32 per hour.

Core Projects And Responsibilities

  • TX Medicaid Cervical Cancer Screenings (Pap Tests):
    • Conduct outreach to Texas Medicaid members to assist with scheduling appointments for Pap screenings, well-woman exams, and other preventive services such as mammograms, annual wellness visits, A1c screenings, and blood pressure checks.
    • Educate members on the importance of these screenings and guide them through the process of receiving care. Medicare Statin Medication Referral Outreach:
    • Work closely with providers to identify Medicare members who may need statin medication added to their treatment plan.
    • Facilitate communication between providers and members to schedule appointments for medication referral and ensure compliance with health care plans.

Work Schedule

  • Standard work hours are 8 am – 5 pm CST, with occasional opportunities for overtime or alternate hours if required.

Essential Functions

  • Design and implement targeted health education interventions based on the needs of the population served.
  • Perform outreach and engagement activities to improve member adherence to preventive screenings and recommended treatments.
  • Collaborate with internal departments, community organizations, and health care providers to extend resources and develop referral pathways.
  • Monitor and document progress toward health education goals, providing regular updates and reports.
  • Utilize and maintain databases for tracking member outreach, appointments, and follow-ups.

Knowledge, Skills, And Abilities

  • Strong verbal and written communication skills.
  • Ability to work independently in a remote setting, maintaining productivity and regular attendance as agreed upon.
  • Highly organized, with the ability to coordinate multiple outreach projects simultaneously.
  • Proficient in building effective work relationships with members, providers, and internal teams.
  • Committed to maintaining confidentiality and complying with HIPAA regulations.

Qualifications

  • Education: Master’s degree in a health-related field or equivalent experience in patient education is preferred.
  • Experience: 0-2 years of experience working with culturally diverse and low-income populations, ideally within a managed care or health education setting.
  • Experience in telephonic outreach and health care communication is a plus.
